Please contact the office for additional information regarding fees for services provided. We accept the following payment methods: private payment, Employee Assistance Program (EAP), and most major insurance provider reimbursement. Sliding scale fees are available for those who qualify. Payment is due at the time services are rendered. We accept cash, check, Visa, Mastercard, Discover, and American Express.

*Illinois Medicaid and Managed Care are not accepted.

Cancellation Policy

It is our policy that you contact our office within 24 hours of your appointment time if you are unable to attend your scheduled appointment. If you do not show for your appointment or cancel in less than 24 hours notice, a No Show/Late Cancellation fee of $150.00 may be charged for the missed appointment. This cost is not covered by insurance and will be your responsibility to pay prior to the next scheduled appointment.
